Rules and Regulations

*All inquiries on “Win $1.5 Million” and “Best Ticket Pricing” are to be made to the licensee prior to the purchase of ticket(s). This opinion is based on information available to the licensee on health care lotteries in the greater Toronto area at the time of printing.

There will be 262,000 tickets printed (250,000 will be available for sale, 12,000 will be awarded as prizes). The total number of prizes to be awarded is 14,605. The total value of all prizes is $3,085,186.92 (inclusive of all applicable taxes and freight). Only ticket(s) that have been paid for, and have been verified as paid for, or won as prizes, are eligible for respective draws. In the event of an invalid cheque or credit card payment, ticket(s) issued will be considered void, and you will be notified in writing.

^◊Group Purchase Program is applied to groups of tickets purchased, whether by an individual or a group of individuals. Tickets are 2 for $100, 3 for $135, 5 for $195, 7 for $245, 15 for $375 and 25 for $500. The Hospital for Sick Children and SickKids Foundation assume no responsibility whatsoever for the entitlement of any prize to any group or group member other than the Primary Purchaser or for the accuracy of any of the information at any time contained on the SickKids Group Purchase Program order form. In the event that a winning ticket is held in more than one name, it is the responsibility, and at the discretion, of the Primary Purchaser to share winning information and prizes with those other persons whose names are also listed on the Group Purchase Program order form. The Primary Purchaser is the person who is identified as such on the Group Purchase Program order form, or otherwise to SickKids, and whose credit card, cheque, or money order pays for the purchase of the tickets.

Tickets may be ordered by mail, phone, fax, in person (at The Hospital for Sick Children), or online at www.SickKidsLottery.ca. Tickets cannot be purchased in the name of a minor. Purchases are limited to persons 18 years of age or older. Tickets must be purchased in Ontario and cannot be mailed outside of Ontario. Tickets are not tax deductible. All purchases will be processed and fulfilled under the supervision of MiraTel Solutions Inc. Purchasers do not need their ticket(s) in hand in order to be included in the respective draw(s).

Winning tickets will be drawn under the supervision of MiraTel Solutions. After each draw selection, winning tokens are recorded and returned to the drum, to be eligible for all other subsequent draws. After every fifth draw selection, the drum will be rotated a minimum of one full rotation. All prizes must be accepted as awarded. No substitutions except where a cash alternative option is specified. In the event you have won a prize with a cash alternative, you will be notified in writing and a waiver form will need to be signed and returned to SickKids Foundation before receiving the prize(s). Prize(s) must be claimed within one year after the final draw date. Prizes may not be exactly as shown.

All draws will take place at the offices of MiraTel Solutions Inc., located at 2501 Steeles Ave. West, North York, ON. Draws will commence at approximately 9 a.m.

**VIP Draw includes one draw for two 2011 “smart fortwo” vehicles or take the cash ($30,000 cash value). Prizes may not be exactly as shown. One prize draw will take place on February 14, 2012. To be eligible for the VIP Draw, tickets must be purchased by midnight January 27, 2012. The VIP Draw is open to all Ontario residents.

Early Bonus Draw includes one draw for a 2012 Chevrolet Corvette or take the cash ($50,000 cash value). Prizes must be accepted as awarded. Prizes may not be exactly as shown. One prize draw will take place on March 6, 2012. To be eligible for the Early Bonus Draw, tickets must be purchased by midnight February 24, 2012.

Early Bird Draws major prize consists of a 2012 Mercedes-Benz CLS550 4Matic Coupe or the option to take the cash ($100,000 cash value). 12,000 free SickKids Lottery tickets will also be drawn. All Early Bird draws will take place on March 20/21, 2012. Prizes must be accepted as awarded. Prizes may not be exactly as shown. To be eligible for the Early Bird Draw, tickets must be purchased by midnight March 9, 2012.

Grand Prize Draw is one (1) draw consisting of $1,500,000 cash, which will be conducted on March 27, 2012. To be eligible for the Grand Prize draw, tickets must be purchased before midnight March 16, 2012. Please allow a minimum of four weeks for delivery of cheques.

7 Dream Pack Prize Draws will be drawn in the following order, on March 27, 2012. To be eligible for the Dream Pack prize draws, tickets must be purchased before midnight March 16, 2012

(1) DREAM PACK #1 – includes a 2012 Audi S5 4.2 Tip Quattro Coupe (valued at $73,222.22) plus a Kensington Tours travel certificate (valued at $20,000) or take $80,000 cash.

(2) DREAM PACK #2 – includes a 2012 Chevrolet Avalanche (valued at $41,814.52) plus a Kensington Tours travel certificate (valued at $20,000) or take $60,000 cash.

(3) DREAM PACK #3 – includes a 2012 Ford Mustang Coupe (valued at $26,463.92) plus a Kensington Tours travel certificate (valued at $10,000) or take $35,000 cash.

(4) DREAM PACK #4 – includes a 2012 Chevy Cruze (valued at $23,508.52) plus Kensington Tours travel certificate (valued at $10,000) or take $30,000 cash.

(5) DREAM PACK #5 – includes a Kensington Tours travel certificate (valued at $20,000) plus $5,000 cash or take $25,000 cash.

(6) DREAM PACK #6 – includes a Kensington Tours travel certificate (valued at $15,000) plus $5,000 cash or take $20,000 cash.

(7) DREAM PACK #7 – includes a Kensington Tours travel certificate (valued at $10,000) plus $5,000 cash or take $15,000 cash.

Vehicle Prize Draw (1 prize draw) There will be one winner with a choice to select from the following vehicles:
2012 Land Rover Range Rover Evoque; 2012 Cadillac CTS Wagon 3.0L V6; 2012 Volkswagen CC Highline 2.0T 6sp; 2012 Camaro SS(V8); 2012 Audi A3 2.0T Progressive S line Quattro; 2012 Acura TL Premium; or take the cash option (cash value is $40,000). Vehicle Prize draw will be conducted on March 27, 2012. To be eligible for Vehicle Prize draw, tickets must be purchased before midnight March 16, 2012.

Final Travel Prize Draws (3 prize draws) are provided by Kensington Tours. Three gift certificates valued at $8,000 each will be awarded. Winners have the choice of any itinerary and/or flights which Kensington Tours provides services to.

Final Draws: (2,590 prize draws) Final draws will be awarded from highest to lowest value. To be eligible for the Final draws, tickets must be purchased before midnight March 16, 2012. Final Prize draws will begin on March 27, 2012 until completion on March 28, 2012.

All winners will be notified by mail with complete instructions on how to redeem their prize. Those winners who have won major prizes may also be contacted by phone. On April 5, 2012, there will be an advertisement in The Toronto Sun announcing all major prize winners. In addition, a complete list of winners will be searchable online at www.SickKidsLottery.ca. A printed list will be available at SickKids Foundation upon request, or ticket purchasers may call 1-888-501-KIDS (5437) to find out if they are a winner. In the event there is a discrepancy between the winners list posted in the Toronto Sun and that posted on www.SickKidsLottery.ca or the MiraTel Solutions final report, the MiraTel Solutions Inc. report will be considered final. Winners agree to the use of their name and photo for publicity purposes by SickKids Foundation. SickKids Foundation respects the privacy of its lottery supporters; we do not sell, rent, or trade our supporter lists. The information we collect is used to process lottery ticket purchases, keep our supporters informed about the activities of The Hospital and Foundation, and ask for their support for our mission to improve children’s health. If at any time you wish to be excluded from such contacts, call us at 416-813-6166, and we will accommodate your request. By entering this lottery, participants acknowledge that they have read and understand the rules and regulations and confirm that they are at least 18 years of age. Approximate retail value on all prizes is as of the date contracts were negotiated with suppliers.

The following persons are prohibited from purchasing SickKids Lottery tickets or obtaining benefit from the prize offering: SickKids Foundation staff, Board of Directors, and Lottery Advisory Committee; The Hospital for Sick Children Executive Team and Board of Trustees; employees of registered gaming suppliers; and members of the same household of each of the above. The liability of the licensee of this lottery shall be limited to the purchase price of the ticket.
